My Pregnancy Notes

Update to Patients: 01 February 2021

Thank you to everybody who has signed up to the 'My Pregnancy Notes app'. We've been delighted to see so many of our patients register.

Our hospital team are processing all registrations as quickly as possible, due to the great take up of this system it is taking a little longer to complete registrations - so please bear with us.

We would also like to highlight that if your appointment was prior to the launch of the 'My Pregnancy Notes app' on Tuesday 19th January 2021, you may still register for My Pregnancy access but your appointment information will not be held within the portal, it will remain in your hand-held white folder. ONLY patients who have attended after the 19th January will be able to view appointment information in the 'My Pregnancy Notes app'.

My Pregnancy Notes

All pregnancies booked to have their baby at Liverpool Women’s Hospital are now able to access their Maternity Notes online.

This web-based portal will allow you to view your Maternity Notes securely online, from booking until being discharged from Maternity care. This tool allows you to access your notes wherever and whenever you wish and communicate better with your clinicians.

My Pregnancy Notes will allow you to be more involved in your care and provide your clinicians with more information than ever about you preferences. It will enable you access to up to date information about your care and the leaflets and information you need, whenever you need it.

This new online service will enable you to:

  • Record your Personalised Care Plan 
  • Note down any questions you wish to ask your Midwife at your next appointment
  • Add notifications, for example changing GPs or moving away from area
  • Access leaflets and information
  • Add Your Maternity Exemption Certificate and MAT B1

You can register for My Pregnancy Notes following your initial booking appointment.

Please note it may take up to 48 hours for your records to be merged.

You will still need to bring along with you your paper GROW chart to every appointment.

When does this launch?

The system was launched on Tuesday 19th January 2021, for all Liverpool Women’s maternity patients.

FAQ's

  • Currently pregnant, booked at and intending to birth at LWH

    If you are currently pregnant and are carrying a hand held set of notes, from Tuesday 19th January, if you have an antenatal appointment, it will be completed in our new electronic system and you will be able to register for access via mypregnancynotes.com. This will allow you to access your notes as you do your handheld records. You must still bring your file with you to all appointments.

     

  • Birthing elsewhere

    If you are receiving antenatal and postnatal care from LWH but intend to deliver elsewhere, you can grant the hospital you choose to birth at third party access to your case notes.

     

  • Birthing at LWH

    If you are receiving antenatal and postnatal care at another Trust, but are planning to birth at LWH, you will continue with the records that you currently have, and we will print your delivery summary and discharge paperwork for your community midwife.
